‘How will my new hairline look?’ is one of the most important questions patients ask us when they begin our consultation. ‘Will it look rounded, symmetrical, asymmetrical, more defined or less defined?’ It is a deep desire to want to know how the cosmetic procedure will look finished before it starts. People have a strong opinion and idea of how they want their new hair to look and move.

The truth of the matter is there is no one size fits all hairline that Dr. Yazdan, or any top hair transplant surgeon for that matter, tries to create. What looks good for one patient may not look good for another – what makes sense for a young patient with progressive loss may not make sense for an older patient with more ‘stable’ and limited hair loss. In other words, hairlines must be carefully considered and customized for each patient.

For this patient, who likes to wear his hair short and based on his age, level of hair loss, hair-type and forehead shape, we decided to design a flatter, more defined, less irregular hairline. Dr. Yazdan use a very calibrated small punch during the FUE procedure to minimize scarring and maximize a natural look.

For this older patient, with lots of wispy strands and excessive hair loss who wears his hair longer, Dr. Yazdan decided to design a slightly more conservative, soft, broken-up, irregular, and feathered hairline. Patient had arrived at Modena Hair Institute more than a year ago with a poorly performed punch surgery and lots of scarring. He had also received scalp micro pigmentation work which turned blue. A notorious problem among inexperience or purely cosmetic clinics, SMP pigments can turn blue if it is too dark and the wrong pigments are used.

Conclusion

As you can see, these patients were about as different in hair type, age, density and shape as you can get. The first patient had coarser, darker, denser and a more limited hair loss experience. The second patient had softer, wispier hair and a greater degree of hair loss. Designing a hairline shape for both patients requires a very different aesthetic and scientific approach.

Patients must visit clinics who are able to showcase a wide variety of hairlines, not a ‘stock’ hairline done time and time again on each patient. A densitometer is a handheld device that delivers digital, magnified images of your hair displayed on an LCD screen, revealing the properties and the diameter of your hair shaft.  

This analysis will reveal abnormal levels of miniaturization, which are decreased hair shaft diameters due to DHT, the hormone that plays a primary role in the development of female or male hair pattern baldness. Restoring Crown Hair Requires a Highly Skilled Surgeon

In the exact same specialized way, believe it or not, restoring hair on the crown requires a different surgical approach to temple or other hair restoration.

A highly skilled surgeon must take a vastly different approach in restoring follicular density to crown hair than to hair on the front temple or sides. 

Unlike the rest of the scalp, hair grows in a circular pattern in the crown, commonly referred to as the whirl or swirl. Many average doctors can transplant hair, but finely and sensitively creating incisions in order for hair to appear both dense and natural is an art form that only a handful of outstanding surgeons possess. 

The hair transplant surgeon must create tiny incisions placed strategically at varying angles that mimic the natural whorl. Only a specialist – not a technician or general cosmetic surgeon – can achieve masterful results.

Simply creating random incisions, even if they are placed closely together, might grow hair, but it often may not look natural, can result in styling restrictions and is often blatantly clear to observers that those sprouting and uneven hairs are not your natural hairs.

Four to six weeks later, most if not all of the transplanted hair in the crown will shed in most cases. Diet, Hair Products and Hair Surgery

‘You are what you eat.’ We’ve all heard that old saying, but how does it apply to the health of your hair? Can your dietary choices lead to hair loss? Can they counter it? Reverse it? 

The primary causes of hair loss are genetics, hormonal dysregulation, hypothyroidism, tension on the hair (tension alopecia), scalp trauma, infection and dietary factors such as nutrient deficiencies.

The average growth rate for human hair is typically around 0.35 mm per day. Meanwhile, the average scalp sheds about 100 hairs per day, which can increase with hair care practices like excessive shampooing and forceful brushing.

Stress, Depression and Androgenic Alopecia

The last few months have not been the rosiest time in the history of human happiness, has your hair come out the worse for it? Stress and depression are often linked with hair loss in popular perception- Is this accurate? And, if so, how does this information apply to your hair loss?

The logic behind the thinking seems routine: stress causes hair loss- hair loss causes stress- stress from hair loss causes more hair loss. And so goes the vicious circle…

Depression often accompanies stress. The emotional distress of depression is frequently coupled with physical symptoms: fatigue, aches and pains, and digestive issues, and at times – hair loss. 

However, while scientific studies have yet to find a direct correlation between stress and/or depression with permanent hair loss, depression and stress may play an indirect role in the thinning of hair. It is important to understand the distinction between stress based hair loss and Androgenic Alopecia, also known as Male Pattern Baldness. Androgenic alopecia is a genetically inherited condition that affects up to 60% of males by the age 50. Therefore, it’s symptoms will manifest based on genetic factors and not lifestyle choices. However, it is important for a sufferer of Androgenic Alopecia to understand that hair thinning caused by stress and depression will not do much to improve the condition of an already balding scalp.

The best way to reduce stress related hair loss is, of course, to reduce one’s stress. Spend more time outside or with friends and family, read a good book, take up skydiving.

Oils and Rogaine Are No Replacement for Hair Transplantation

Perhaps you’ve heard about the benefits of applying moisturizing oils to your scalp in an effort to stop, reverse or prevent baldness? Now, it does seems like there could be something here, afterall we all know that clean and hydrated hair is healthy hair. Does this old folk remedy have any real efficacy and if not, what solutions are known to effectively treat hair loss? 

Olive oil has been a staple of hair care since antiquity and a recent study has provided some intriguing support for its role in promoting healthy hair growth. The Department of Food and Nutrition at Yonsei University, South Korea, identified oleuropein as the hair-stimulating compound in olive oil. The topical application of olive oil (on rodent test subjects) did result in an increased rate of hair growth and follicle size.

Now, what does this mean for somone worried about a receding hairline?

As nice as it would be to simply be able to rub some oil onto your scalp and then suddenly regrow a full and youthful head of hair- things don’t quite work that way. 

Technically speaking – It is certainly true that the application of topical oils may aid in improving the appearance of existing hair, however Androgenic Alopecia or Male Pattern Baldness (the most common cause of hair loss in men) is a genetic condition. 

Applying salad dressing to your head is never going to rewrite your DNA. ‘I’m Literally Going Bald with Stress’

Dr. Yazdan spoke with a young man on the phone this week who has concerns about his thinning hair. He is 33 years old and was very troubled about the diffused thinning on the back of his head and the growing thinness in his crown area, and he wanted to know if a hair transplant procedure would be the best solution for him.

Successful hair transplant surgery depends upon a multitude of factors, including how much hair the prospective patient has on their head at the time of surgery, as well as their age, and potential future hair loss. Only a specialist with years of experience can examine and track these various factors with clarity and tact.

In this young man’s case, one of the major determining factors was how much space there was between the existing hairs on the top of his head and the crown area where he is noticing thinning occurring. Why would this be a determining factor, some may ask? If there is significant gaps and spaces between his existing hairs, then hair transplant surgery may be a viable option. However, if the spacing and gaps between his existing hairs is very small, then hair transplantation could cause damage to the existing healthy hairs through a process known as shock loss. 

While hair transplant surgery offers amazing hair restoration potential and excellent results, there is always the potential for post-operative ‘shock loss’ to the patients existing hair. Given the normal cycles of hair growth – anagen (hair growth), catagen (resting), and telogen (hair shedding) – any hair follicles that would normally continue to grow for a year or more will more than likely regrow following shock. 

Hair follicles that genetically would normally stop growing within the next several months, or are in the telogen phase probably will not regrow. For this reason, an in-person hair and scalp analysis is necessary to determine the best course of action with respect to planning successful hair restoration on an individual patient-by-patient basis.
