Hair Transplant Limitations: Is 5,000 Grafts Too Much?

Great stoic philosophers like Seneca liked to point out that moderation was the key to attaining harmony in the self – ‘a sound mind in a sound body.’ This tidbit of ancient Roman wisdom also holds true for hair transplants. One of the more common questions that our patients tend to ask during consultations is: “Doctor, can we do more?”
We understand you completely. Hair transplants are a big deal for our patients, most of whom (very understandably) want to do as much as possible in a single session, obtain optimal results, and then never have to undergo another surgery again. So back to the question: is more better? Would a massive single transplant of, for example, upwards of 5,000 grafts ever be a good idea? Does having more grafts indicate a better quality transplant? Would using more grafts than recommended and packing them closer together produce thicker, more aesthetically pleasing results? The answer to these closely related questions is- not necessarily. As often happens in medicine, and in life, the answer isn’t always that simple.
Technically speaking, most patients simply don’t need 5,000+ grafts in one go. To give you some perspective, it would take about 5,000 – 6,000 grafts to cover every inch of an entirely bald scalp with adequate density. Most patients end up in our clinic far before that level of hair loss has set in. Most common cases of hair loss involve isolated thinning restricted to certain portions of the scalp. The standard recommended treatment of such cases tends to call for around 2,500 grafts to 3,000 grafts.
Consider this, if it takes 5,000 grafts to cover the entire scalp, we shouldn’t need more than 2,500 or 3,000 to treat thinning of the frontal scalp. Anything more would be simply unnecessary. In fact, it would be very undesirable to force in twice as many grafts as required into a given area; just as it would be highly medically inadvisable to take such grafts from the donor area and then not use them. The donor area only possesses a limited number of grafts and when they’re gone, they’re gone. Hair transplants only address areas of the scalp that are currently thinning or balding. The grafted hair, under normal circumstances, will remain in place for the rest of your life; however, hair transplants do not prevent hair loss in untreated areas of the scalp. Therefore, you may very well want to save some of those extra potential grafts for later.
But what if yours is a case of advanced hair loss calling for around 5,000 to 6,000 grafts to treat? Can we do 5,000 in one sitting then? It depends on several factors. If such a large number of grafts can be safely harvested in one sitting, then certainly. There are some important caveats here though. Firstly, most clinics are simply not equipped to perform a surgery of this magnitude. Secondly, only the FUT (Follicular Unit Transplantation) procedure would be appropriate for such large cases. FUT also known as the ‘strip’ procedure is somewhat invasive and should only be attempted by highly qualified surgeons in a state of the art clinic. Attempting this level of FUT at a clinic which lacks the adequate experience and resources can only lead to disastrous results. Scarring, destruction of the donor area and of the highly sensitive harvested grafts, risk of infection and/or poor final results are all serious risks involved when practitioners and clinics are pushed past the limits of their facilities and expertise.
Moreover, when considering a massive transplant many factors also depend on the individual patient. The average patient is not physiologically capable of safely rendering 5,000+ grafts in one sitting. The laxity (that is, the potential for movement or the ‘stretch capacity’) of the typical human scalp and/or the donor area density often prevents this. Unfortunately, some unscrupulous clinics will go ahead with a 5,000+ graft transplant anyway. As mentioned above, more often than not the results aren’t pretty.
Certain rare patients do possess a broad donor area and excellent laxity. We can safely harvest a reasonably sized strip from them for use in large grafting procedures. The scalps of most patients, however, can only safely provide between 2,500 – 3,000 grafts at one time. Patients in need of 5,000 – 6,000 grafts to cover the whole scalp would simply need to undergo two procedures instead of one. For advanced hair loss, this eliminates the unpleasantness and many of the complications associated with massive grafting procedures and is how the vast majority of such hair restoration procedures are carried out. Anything else would be an exception, highly qualified by individual factors, and most certainly not the norm.
It is also important to let the scalp heal and not to push too much surgery on it at one go. Scalp laxity tends to return within 12 months of surgical recovery by which point it is perfectly safe to undergo a second procedure. As an added perk, due to our highly specialized grafting technique you will still only ever have one incision scar no matter how many times we harvest from the donor area.
Finally, to address the last question: ‘does putting in more grafts than required produce thicker and more aesthetically pleasing results?’ The short answer is a definite no. There are serious and potentially detrimental consequences associated with purposeful  excessive grafting. Here at Modena Hair institute we practice dense graft packing to achieve a full and healthy restoration; however our highly trained and specialized professionals understand that the grafts can only be placed so close.
More grafts do not necessarily produce a better appearance and, furthermore, a region of the scalp subject to excessive incisions will suffer inevitable and irreversible tissue damage. In fact, such tissue damage can even lead to necrosis and infection killing the new grafts and leaving the unfortunate patient with some very nasty scars. There is also no guarantee that badly damaged tissue will be receptive to future grafting. So to go back to our Roman philosophers- moderation is key. A smaller number of judiciously placed and safely harvested grafts, in the hands of our experienced surgeons, will always yield better results than an ill-conceived and equally ill-advised ‘mega graft.’

How Long Will My Hair Transplant Last?

A hair transplant will give you a beautifully restored head of hair, but does it last? Here at Modena Hair Institute we occasionally hear from concerned prospective patients who, while admiring the fine results of our finished hair transplants, are nonetheless concerned that, given enough time, their newly restored hair will eventually thin and fall out. The good news for you is that this is absolutely not the case. Transplanted hair follicles are harvested from a region of the scalp (called the ‘permanent zone’) that is genetically resistant to hair loss; therefore, most patients can expect their newly implanted hair to last forever.
The two most commonly employed hair restoration procedures — Follicular Unit Transplantation (FUT) and Follicular Unit Extraction (FUE) are intended as lifelong solutions to hair loss. The two methods work like this:
Follicular Unit Extraction (FUE) – In FUE individual hair follicles are removed from a donor area of the scalp and are transplanted to the area in need.  No stitches or sutures are required for this minimally invasive procedure.
Follicular Unit Transplantation (FUT) – In FUT a strip of scalp tissue is removed from the donor area. The scalp tissue is then separated into individual grafts for the needed areas of the scalp.
The transplant should grow as hair normally. In both the FUE and FUT procedure transplanted hair is harvested from the permanent zones— also known as the donor area — on the back and sides of the scalp and skillfully transferred to the thinning and balding areas of the front, top, and crown.
It is, however, important to remember that hair transplants (like any other surgical procedure) are not without certain caveats. The results furnished by a hair transplant may vary depending on a number of factors and limitations. Although  the vast majority of  hair restorations do provide natural-looking and healthy results, some limiting factors to consider include:
Choice of clinic: Techniques employed for hair restoration procedures have improved dramatically over the years and it is essential to find a clinic whose practitioners are well informed regarding all of the latest developments in the field. Top-rated clinics consistently achieve a large graft to growth success rate. Unfortunately, many sub-standard clinics exist. Moreover many so-called ‘bargain’ hair transplants, especially those performed overseas or by underqualified practitioners, can be downright dangerous and leave patients with severe scarring. Doing your own research, exercising good judgement and reading written testimonials of former patients is essential. Many surgeons consistently deliver effective and beautiful results; do your due diligence to ensure that you are in good hands before you go under the knife.
Biological Factors: We must accept that hair transplants do not work for everyone, nor are they a miracle cure. Advanced hair loss, for example, can be particularly difficult to address; patients whose hair follicles have remained dormant for two to seven years and/or those with particularly thin hair in the donor area will often experience less satisfactory results. Of course, this doesn’t mean that dramatic cosmetic results are impossible to achieve, just that initial expectations should be set lower. In most cases some well placed transplant hairs produce a far more pleasing, vigorous and youthful aesthetic effect than no hair at all.
Continuing Hair Loss: While the transplant itself should keep growing forever, the same can not necessarily be said about the rest of your hair. While hair transplants do not prevent future hair loss in regions of the scalp not covered by a transplant, they do help cover them. Effective treatments for continuous hair loss exist and our specialists are always happy to discuss any concerns or anxieties you may have regarding our treatment options.
Adequate Donor Area Capacity: The size of the permanent zones vary according to individual genetic factors. A larger donor area allows for more grafts to be harvested and furnishes our practitioners with a greater number of hairs to work with for the transplant. Patients with relatively small permanent zones should modify their  expectations in regards to the amount of coverage that can be achieved. However, as mentioned previously, truly skilled transplant surgeons can still often produce impressive results even when confronted with sparse material to work with.
Shock Loss: Transplant hair may fall out due to shock shortly after surgery. This is a normal and expected physiological response and nothing to be alarmed by. The new follicles remain and you should expect to see significant growth within the course of a few months.
Both FUE and FUT are permanent procedures. The donor area of the scalp, from which hair follicles are harvested, isn’t called the ‘permanent zone’ for no reason. Hair harvested from this region of the scalp is genetically programmed to never fall out. However, while some patients who receive a hair transplant will later opt for another due to further thinning of non-treated areas, you can rest content knowing that, under normal circumstances, the direct results provided by FUE or FUT are there to stay forever.

A Complete Guide to Hair Transplantation Scarring

Anxiety is a completely natural and reasonable response to the prospect of undergoing any surgical procedure. For this reason it’s not surprising for our patients considering hair transplant surgery to occasionally express their concerns about the possibility of scarring.  Hair restoration surgery is after all an elective form of treatment undergone for aesthetic purposes; therefore it stands to reason that excessive scarring would somewhat defeat the purpose of such a procedure. Firstly, it is important to understand that no surgery known to modern medicine carries a 0% risk of scarification. In fact, various medical oversight bodies have issued official statements condemning those who advertise any sort of surgical procedure as ‘scar free.’ However, despite these natural caveats, hair transplants remain (with good reason) an immensely popular form of treatment the effects of which, on an individual’s general wellbeing, far supersede the merely aesthetic. And we confidently assert that our clinic’s surgeries leave scarring so fine that it is practically imperceptible unless under a microscope.
To help alleviate any anxiety you may have, let’s take a closer look at hair transplants and scarring.
Scar tissue forms as part of the body’s natural healing response and, in a surgical context, is proportional to the extent and depth of the incision. Any damage to the skin can result in scarring; even a paper cut can leave subdermal scar tissue. During a commonly used technique for hair restoration surgery there are two areas of the scalp on which small incisions are made; donor area, from which the hair transplant grafts are harvested, and the recipient area where the same hairs are transplanted.
The small slits made upon the bald part of the scalp (the recipient area) are made with minimally invasive surgical implements and, if done correctly by a skilled professional, should result in no visible scarring. Subdermal scarring will not be visible. Visible scarring may result in certain cases if, for example, large implements are used or if a practitioner attempts to squeeze an injudiciously large amount of grafted hairs into the skin of the scalp. Again, while it would be disingenuous to suggest that there any procedure is 100% scar free, as there will always be some changes below the skin, under normal circumstances you should expect the skin of your scalp’s recipient area to look more or less the same as before the procedure; except, of course, that it will now be covered by a beautiful new growth of hair!
Very minor scarring in the donor area is, however, unavoidable with any currently known medical procedure. Any practitioner who suggests otherwise is, quite frankly, lying. The hair grafts must be removed by means of judiciously placed surgical incisions and a cut, by its very nature, will inevitably leave some form of scarring. The good news for you, however, is that such scarring as is expected to occur during the harvesting of hair follicles is minimal. The incisions made by highly trained professions on the donor area using the highest quality surgical implements are difficult to see once recovery is complete and will also be hidden below hair. To better understand scalp scarring in the donor area we need to discuss the two different methods for harvesting hair follicles; Follicular Unit Transplantation, also known as FUT and Follicular Unit Excision, or FUE.
Follicular Unit Extraction (FUE) – In FUE individual hair follicles are removed from a donor area of the scalp and are transplanted to the area in need. No stitches or sutures are required for this minimally invasive procedure. Tiny implements, some of them measuring less than 1mm, are used to carefully excise individual hair follicles, small forceps are then used to free the graft. The small holes left behind from the cutting and removal of the grafts fill with scar tissue, leaving small dot-like scars wherever a graft has been extracted. When performed correctly, FUE scarring is almost impossible to discern.
Correctly is, however, the operative word – to minimize visible scarring the number of extractions must be small. In an inappropriately performed FUE procedure thousands of these small dot-like scars will be noticeable which, along with visible thinning of the donor region from overharvesting, is not an ideal aesthetic situation. The appearance of scar tissue varies somewhat with skin tone. As scar tissue is often lighter in color compared to one’s natural skin tones, patients with darker skin may have more apparent scarring from FUE because of a greater degree of contrast. In either case though, the scars are difficult to notice even on those who prefer very short hairstyles; if they so choose, our FUE patients should easily be able go down to a #2 (1/4th of an inch) on the clippers while retaining excellent scar coverage.
Follicular Unit Transplantation (FUT) – In FUT a strip of scalp tissue is removed from the donor area. The scalp tissue is then separated into individual grafts for the needed areas of the scalp. The resulting FUT scar is also a very fine, straight line in the donor region. Typically less than 1mm in width although patients with certain physiological characteristics may develop a scar up to 2mm in width. Both 1mm and 2mm scars are considered to be within the normal range of healing. Your small scar will be hidden by any reasonable length of hair growth. Hair grows above, below and through the scar. Hair growth through the scar is facilitated by a special closure technique; other techniques also exist that ensure, for example, that you will only ever have one scar even if multiple harvestings are required. In all typical cases the scar heals so well as to be indistinguishable even on those who prefer to wear their hair in very short styles. We have observed that even patients with hair buzzed down to as short as 3/8ths of an inch using the #3 guard on a set of barber clippers still have sufficient hair to render the scar barely discernible. The small linear incision scar resulting from FUT provides our practitioners with the greatest number of available grafts. The harvesting process causes no stress to the grafts themselves and, moreover, no further damage or scarring in the donor region. This guarantees that, if required, more donor hairs will be available for future surgeries.
Now let’s talk about subdermal scarring. Scarring beneath the surface of the skin occurs with both the FUE and FUT procedures. Owing to the nature of these procedures and also to the body’s natural scarification process,the subdermal scar is almost always going to be bigger than the visible surface scar. This is not an issue with FUT as the 1mm-2mm scar typically has only an additional 2-3 mm of  scar tissue below the surface. Thus hair follicles within millimeters of the FUT scar are totally untouched and ready for future harvesting.
On the other hand, FUE scars may form too close together if the procedure is not performed correctly. As we mentioned above, FUE cuts made too close together are to be avoided. Such overharvesting may result in an overlapping network of web-like  scar tissue (sub-dermal fibrosis) compromising the health of surrounding hair follicles and rendering further harvesting problematic. The small surface scars from a properly performed FUE procedure are, again, difficult to appreciate even on those with quite short hair.
While most patients are left with minimal, well hidden scarring (and a fully restored head of hair), the technique of Scalp Micropigmentation (SMP) can be performed to further minimize scarring if necessary. SMP is quite similar to standard tattooing, although it utilizes natural pigments in place of ink. SMP involves tattooing small dots which look like shortly buzzed hairs onto the scalp which help to darken, camouflage, and break up the appearance of scar tissue thus allowing patients who wear shorter hair to effectively hide any trace of visible scarring.

Weight Gain and Hair Health

Now, we all know that genetic factors can lead to hair loss, but there are other less obvious potential causes of hair loss which may surprise you. Some of our patients have expressed their concern regarding a possible correlation between hair loss and weight gain. The fact of the matter is, surprising as it may seem, hair loss and weight gain can indeed be correlated. The body is an almost infinitely complicated natural wonder; a negative change in any one part of the body’s system can easily yield unexpected results in another, superficially distinct, part of that same system. Often, weight gain can trigger a host of serious health and wellness issues and, as we have discussed in our previous posts, an unwell body is more likely to suffer from hair loss.
However, the good news about hair loss caused by health and lifestyle problems is that surgical hair restoration treatment is rarely required. In such cases, lifestyle changes and the medical treatment of preexisting conditions are often sufficient to restore one’s hair to its former state.
Let’s talk about some of these common health issues which may lead to weight gain and hair loss. There is a strong connection between our mental health and our physical well being. Therefore,  it is not surprising that depression has sometimes been shown to be linked with hormonal imbalances. Whether such imbalances are the cause or the result of depression, the fact remains that hormonal state and hair health are deeply connected.
While depression alone can constitute a sufficient cause for hair loss, lack of physical activity can also have a detrimental effect on both general wellness and hair health. Besides potentially leading to depression, a lack of exercise is often the cause of weight gain and obesity, both of which can be damaging to the body and lead to hair loss.
Diet and exercise, as you’ve no doubt heard, are key. If you are not eating a well balanced diet, this can have a severe negative impact on your overall health,wellness and mental state. Eating too much ‘junk food’, particularity foods containing high fructose corn syrup, can be extremely taxing in the body. Strive to eat a well balanced diet high in fruits and vegetables. The high antioxidant content of white and green teas can work wonders for promoting general health, maintaining healthy gut flora and flushing the body of toxins and impurities. A body deprived of the necessary vitamins and minerals it needs to function will inevitably break down, the effects of which may very well become manifest in an accompanying decline in hair health.
Certain pre-existing conditions like diabetes may, over time, lead to a decline in hair health. Diabetes (Type I and II) can lead to damaged blood vessels including those of the scalp. Hair follicles, if deprived of the oxygen needed to nourish and support their well being, are more likely to become damaged in ways that will manifest as noticeable hair loss. As mentioned above hormonal imbalances can lead to hair loss, in particular the condition of  hypothyroidism. Hypothyroidism describes a condition in which the thyroid gland fails to produce the necessary quantity of certain essential hormones. There are a large number of potential causes among which congenital disorders and autoimmune diseases, the side effects of certain medications, the after effects of thyroid surgery or radiation therapy, Iodine deficiency and/or old age may all play a significant role. Whatever the cause, such hormone imbalances in turn have a negative effect on the metabolism. Hormones regulate a host of important bodily functions and conditions like hypothyroidism often result in unpleasant symptoms such as weight gain, depression, fatigue, dry skin, and, of course, hair loss. Hypothyroidism is best treated with medications that specifically target and restore proper hormone levels in the body which, in time, may help to reverse the adverse symptoms of the condition.
As we have mentioned above, lifestyle is key; not only for healthy hair, but for your general well being and state of mind. Our synergistic bodily processes must be maintained by a healthy lifestyle, a balanced diet, plenty of exercise, by engaging in mentally stimulating activities and by avoiding bad habits like smoking and excessive alcohol consumption. Often, the first and most crucial step to healthier and thicker hair is almost always a lifestyle change. By improving your habits and lifestyle you may very well save yourself  the trouble of pursuing a surgical course of treatment as your hair loss may reverse course and mend itself within the course of a few months. Female Hair Loss Success Story: PRP Hair Restoration Following Pregnancy

An exciting new entry has just been added to Modena’s female hair loss success stories!

In the following presentation, Dr. Amir Yazdan’s own sister, Desiree Yazdan – herself a renowned doctor of dental medicine – undergoes PRP treatment for the progressive hair loss she experienced during pregnancy.

Hair thinning and loss during pregnancy is very common, caused by big changes in hormone and estrogen levels which can speed up the natural cycle of hair shedding. Thyroid disorders and iron deficiency arising from such hormonal imbalances can also contribute to hair shedding.

Dr. Desiree Yazdan received four PRP injections applied directly to the problem areas on the scalp, scheduled at one injection every three weeks. After the fourth and final injection, hair growth was significantly restored across the scalp, particularly around the temples. Hair grew back straight, thick and in harmony with her natural hairline.

We’ve touched on the restorative benefits of platelets before, but here we’d like to emphasize the effectiveness of PRP in treating post-partum hair loss. Progressive and Diffuse Hair Loss

Male pattern baldness and women’s hair loss manifest differently and accordingly call for different approaches to treatment. To better understand how different kinds of hair loss call for different kinds of treatment, let’s have a look at two of the most common varieties- progressive and diffuse balding- along with their common causes and some recommended methods of treatment.
Progressive and Diffuse Balding
It’s important to understand that hair loss may present itself in a variety of ways and, as there is no single balding pattern, there is also no single optimal treatment for all cases of hair loss. Thus, it is particularly vital that the individual nature of what you are experiencing is carefully and thoroughly evaluated by our expert diagnosticians. As the old architectural dictum goes, ‘form follows function’ so also the nature of your hair loss will often serve to indicate, to the well-trained eye, both the underlying cause of the hair loss as well as the best option for treatment.
Progressive hair loss, also known as androgenic alopecia, is caused by genetic factors. It is estimated that around 70 percent of men and 40 percent of women are afflicted by this condition. Progressive hair loss can manifest in men at any time starting from their early 20s and will often begin with thinning centered around the hairline and the crown of the head. Progressive hair loss in women can start in the 30s or 40s. However, unlike the distinctive horseshoe like pattern often observed in males, female hair loss tends to occur all over the scalp.
Thankfully, there are a variety of surgical and non-surgical treatment options available for progressive hair loss. Topically applied Rogaine or orally ingested Propecia are two non-surgical methods of hair loss treatment which may be able to help slow the rate of hair loss and to promote fuller hair growth.
