Claire’s Testimony: I contacted Modena Hair Institute because I was so distressed about the way I looked. I had plucked my eyebrows from an early age and unfortunately, they hadn’t grown back. Which One of These is Causing Your Scalp Irritation?

Telogen Effluvium: The Second Most Common Form of Hair Loss

If you’ve been familiarizing yourself with the diverse types of hair loss conditions out there, you may have read the scientific term “effluvium” pondering on what that word even means; effluvium is the ‘scientifically correct’ term for the outflow, and anagen and telogen refer to the distinct phases in your hair growth cycle.
One common misunderstanding amongst most people is that your scalp is always producing hair. We hate to break it to you but, it’s not. In fact, each of your hair follicles go through a cycle consisting of 3 phases in which they grow, rest, fall out, and repeat! Another common myth is that your entire head of follicular hairs all grow at one time, yet, if that were the case – there would be plenty more bald heads bobbling around the universe at a youthful age. Rather, your hair follicles are typically all at a somewhat different stage of the hair growth cycle which consists of 3 phases: Anagen, Catagen and Telogen.
Telogen Effluvium is generally considered to be the second most common type of hair loss. TE is a scalp disorder characterized by the thinning or shedding of hair causing early entry of hair in the telogen phase. The telogen phase, or ‘resting phase’ last for approximately 3 months. Generally speaking, most people will have 10-15% of their hairs in this phase at any given time. Another fun fact: Most people shed approximately 100 hairs a day in this phase! With that said, if you suffer from Telogen Effluvium…you may see an inflation in the amount of hairs you are shedding in a day.
If you are suffering from Telogen Effluvium, you are most likely experiencing hair loss or hair thinning across the entire scalp. So, what causes this affliction?

Hair Growth Cycle and Why it’s Important

Opposed to widespread belief, the human hairs do not all grow at one time. Rather, each individual strand of hair is constantly undergoing one of the three phases to determine whether it should grow and shed. The anagen phase is part of the hair growth cycle or ‘active phase’ when the hair enters a state of active growth. This is the only time new hair is growing. Most of your top hair transplant surgeons would agree that this period lasts approximately 3 to 5 years on average. What Happens During the Anagen Phase?

  • The hair follicle itself begins to enlarge and grow deeper into the scalp
  • When the hair follicle becomes deep enough, a hair bulb forms at the bottom
  • Connective structures called derma papillae fasten themselves to the bulb of the hair follicle providing nourishment so that the hair can grow
  • The hair shafts begin to grow upwards and outwards from the scalp. If an old hair is still present, the new hair shaft pushes it up and out of the follicle
  • The new hair shaft grows upward through the follicle, past the surface, and emerges as a hair strand

Is the Anagen Phase the Same for Everyone?
The duration of the anagen phase is mainly determined by genetics. For example, those of Asian descent generally have hair that remains in the growth phase for a considerably longer duration than other ethnicities. In some cases, scalp hair remains in this phase as long or short as their body hair. This results in scalp hair not growing more than just a few inches.     • Hairline is too straight (vertical) – This being a cardinal sin.     • Unnatural grafting – this one combines a host of technical errors that include too large of grafts (i.e. 4 hair grafts or more) in the front hairline, the angles of the grafts being improperly too high, pitting from placing the grafts too deep, grafts being squeezed down causing compression because the grafts do not fit in the previously made sites.
    • Punch size – in terms of hair restoration, size matters! Punch size refers to the size of the tool used in an FUE (Follicular Unit Extraction) in order to harvest grafts from the donor zone. Larger punch tools require larger sites to be made at the recipient area, in this case, your risk of scarring can increase dramatically. Many physicians either don’t feel competent enough to perform a successful hair transplant using smaller punches or they find it easier and quicker to use a larger punch instrument when extracting thousands of grafts, making it less taxing on them, yet, your paying the price with less than satisfying results! You’re risk of scarring goes up, while your grafts success rate goes down. Many physicians label their punch tools ‘A & B’ so you can’t see the size they’re using. Don’t let them fool you! Dr. Yazdan is one of the few hair restoration surgeons out there that are willing and able to get clean, crisp 1-2 hair grafts easily using some of the smallest punches.

  • Shape and position of the hairline is off– when the shape of the hairline (made by the physician) does not match the normal shape of a man or woman’s hairline, it can be a dead giveaway that a hair transplant was done. In addition, if the physician positions the hairline down too far (i.e. the forehead), this can also prove to be an aesthetically unpleasing appearance. Hairlines should almost always rest on the transition from the forehead to the scalp and not any lower. Another common mistake inexperienced physicians tend to make is not evaluating the hairline from the side profile as well. The slope of the hairline should taper off as the hairline goes back and if not done correctly, can be another dead giveaway.
  • Hairline doesn’t match the temples – when the hairline shape and the degree of recession are not matched, your results could look very unnatural. Rebuilding the temple to match the hairline can be an important part of the hair transplant procedure. Forcing the hairline too far anteriorly would mismatch the degree of the temple recession.
  • Hair plugs – let’s not forget to mention the use of hair plugs back in the day. The unsightly appearance of hair plugs… the fundamental problem with the unnatural-appearing hair graft is the graft size. Hair plugs use upwards of 20 or more hairs in a single graft which meant using a punch instrument of 1.5-1.7mm.
